Below is a list of forms that are required to attend camp.
All campers (both youth and adult) are required to complete a health history and authorization form in order to attend camp. Please notify us in advance if you will require special accommodations to attend camp. Individual arrangements can be made as needed.
A Medical Information Form can be submitted in one of two ways:
Adult campers over age 18, please use the ADULT Medical Information Form and the ADULT Medication List.
(Important: If you have a food allergy or other concern we should be aware of before camp starts, please mail your form directly to camp at least 2 weeks prior to the start date of your event.)
Medication is any substance a person takes to maintain and/or improve their health. This includes vitamins and natural remedies. All medications are collected, stored, and distributed by camp health care personnel. Please list ALL medications through your online account Dashboard. (Include prescription, over-the-counter, or non-Prescription drugs taken routinely.) Bring only enough medications to last the entire time at camp. Keep it in the original packaging/bottle that identifies the prescribing physician (if a prescription drug), the name of the medication, the dosage, and the frequency of administration.

All camps require a Release of Liability Waiver and Public Relations Release for participation in any Dakotas United Methodist Camping event.
